ABSTRACT

Happle-Tinschert syndrome is a disorder causing unilateral segmentally arranged basaloid follicular hamartomas of the skin associated with ipsilateral osseous, dental and cerebral abnormalities including tumors. Although a case with hemimegalencephaly was previously described, this is the first report of Happle-Tinschert syndrome with discrepant short left leg, ipsilateral skin lesions, hemimegalencephaly and frontal polymicrogyria.

ABSTRACT

BACKGROUND: The hemiepiphyseal stapling has both positive and negative effects on effective leg length. The purpose of this study was to analyze change in effective leg length after angular correction by hemiepiphyseal stapling, and to validate in clinical cases. METHODS: Mathematical analysis of a hemiepiphyseal stapling model was conducted. The induced formula was validated in 6 cases fulfilling the assumptions of the model. Anatomical parameters involved in this formula were measured in additional 21 cases undergoing hemiepiphyseal stapling or hemiepiphysiodesis. RESULTS: Effective leg length increased or decreased according to three parameters in this model: 1) limb length distal to the operated physis (L), 2) width of the operated physis (d), and 3) the amount of angular deformity to be corrected (theta). Actual change in effective leg length of 6 cases similar to this model coincided with the predicted change at least in its direction. L/d ratio was 4.82 +/- 0.51. CONCLUSIONS: Considering the narrow range of the L/d ratio, hemiepiphyseal stapling is likely to decrease effective leg length if the amount of angular correction is less than 10degrees, whereas to increase it if the amount of angular correction is larger than 16degrees. This should be taken into consideration when selecting the surgical method for angular deformity correction in skeletally immature patients.

Se realizó un estudio descriptivo-observacional sobre el uso del fijador externo RALCA en niños mayores de 6 años con diagnóstico de discrepancia de longitud de los miembros inferiores mayor de 2 cm , utilizando como técnica quirúrgica la osteotomía de la tibia a nivel metafisiario asociado a osteotomía del peroné, tratamientos aplicados en 14 pacientes que acudieron con esta afección a Consulta Especializada de Ortopedia del Hospital Pediátrico de Matanzas, durante el primer semestre del año 2008. El objetivo fue conocer la distribución de diferentes variables clínicas en estos pacientes así como analizar la eficacia del tratamiento para lo cual se realizo una valoración clínico-radiológica previa y la evaluación posterior a la aplicación del tratamiento. La evaluación de la asimetría entre las extremidades se realizó mediante la mensuración clínica desde la espina iliaca antero superior al maleolo interno y el método del escanograma de Kunkle y Carpenter. Se pudo conocer que las causas más frecuentes que originaron la afección fueron las anomalías congénitas y que el método empleado posibilita una corta estadía hospitalaria, alcanzándose una evolución satisfactoria en el 100 por ciento de los pacientes tratados, presentándose escasas complicaciones, y obteniéndose una recuperación total de la longitud del miembro en un tiempo no mayor de un año en la mayoría de los casos.


We carried out a descriptive-observational study on the usage of the external fixer RALCA in children older than 6 years old diagnosed with discrepancy of low members length bigger than 2 cm, using as surgical technique osteotomy of the tibia at the metaphysiary level associated to fibular osteotomy, treatment applied to 14 patients assisting to the Orthopedics Specialized Consultation of the Pediatric Hospital of Matanzas with this affection, during the first semester of 2008. The objective was determining the distribution of different clinical variables in these patients and also analyzing the treatment efficacy. For that we made a previous clinic-radiologic evaluation and an evaluation after the treatment. The evaluation of the asymmetry between limbs was made by clinical measuring from the upper-anterior iliac spine to the internal ankle using the Kunkle and Carpenter's scanogram method. We concluded that the most frequent causes of the disease were congenital anomalies and that the used method allows a short staying in the hospital. 100 per cent of the patients had a satisfactory evolution, with scarce complications, obtaining a total recuperation of the limb length in less than a year in most of the cases.

ABSTRACT

A escanometria pelo "método de Farill" é exame rotineiro na maioria dos serviços radiológicos. Ela permanece, há mais de meio século, como um método amplamente utilizado para diagnóstico da diferença entre os membros inferiores e seu respectivo tratamento pelos especialistas de diversas áreas. Contudo, detalhes na técnica do exame e na avaliação das medidas costumam ser ignorados ou negligenciados, comprometendo o resultado final. Este trabalho tem por objetivo divulgar os detalhes preconizados pelo autor, restaurando a precisão do método, bem como discuti-lo em relação aos demais métodos.


Orthoradiographic measurement employing the "Farill technique" is a routine study in the majority of radiological services. For more than half a century, this method has been widely utilized by specialists in several areas for both measuring and treating differences in length between lower extremities. Nevertheless, technical procedural details during examination and measurements evaluation have usually been neglected or ignored, affecting the final results and consequently the effectiveness of this method. The present study is aimed at publicizing the details standardized by the authors, restoring accuracy to the technique, besides discussing it in comparison with other methods.

ABSTRACT

Se estudian 500 pacientes que presentan lumbalgia crónica de más de cuatro meses de evolución de los cuales pacientes 298 fueron hombres (59.6 por ciento) y 202 mujeres (40.4 por ciento); la edad varió de 15 a 75 años como promedio 30 años; se excluyeron a los que a su ingreso presentaban radiculopatía. Talla 1.60 m. El rango de normalidad de la dismetría varió de 4-24 mm. Se encontró en 394 pacientes (79 por ciento) y la izquierda 106 pacientes (21 por ciento). En todos los pacientes se les realizó proyecciones radiológicas anteroposterior y lateral de columna lumbosacra de pie y descalzo en placas de 14 x 17 pulg. y una escanometría para efectuar la medición de la dismetría. Radiológicamente en todos predominaba el basculamiento pélvico y una escoliosis no mayor a los ocho grados. A todos los pacientes con dismetría mayor de 4 mm, se les indicó tratamiento conservador a base de analgésicos, antinflamatorios, medidas higienico-dietéticas, y la corrección de la dismetría por medio de una talonera que equilibrara el acortamiento, (menos 4mm) y posteriormente con aplicación de ejercicios para la reeducación muscular, se integraron a sus actividades cotidianas, el seguimiento de los pacientes es de seis a 18 meses de evolución, siendo cada vez más satisfactoria la misma. Unicamente el 5 por ciento de los pacientes evolucionaron mal cuando se agregó otra patología extra-articular
